About Us
The nonprofit organization Your Story Concierge Service Inc, was founded by a nurse, Ebony N. Story, DNP, RN, who wanted to fill the gaps for populations outside of the healthcare setting. The organization was founded in 2018 and has been giving back to the community by providing ice cream socials, Christmas parties, nail painting parties, and super bowl parties for the residents of several nursing homes throughout the Central Savannah River Area (CSRA). The organization does not limit its services to the elderly, it also offers education classes for adolescents about issues teens are facing, feeding the homeless, providing scholarships to healthcare professionals, and recognizing.
Through donations, in 2021 the organization gave it first scholarship to a nursing student to help achieve a life long goal. The organization seeks to recognize those outstanding healthcare providers in the CSRA. This need led to the development of the first annual Carolyn Rose award to be given to an outstanding long-term care nurse.
The organization's mission is to offer support and resources to those in need while trying to help build solid futures one story at a time. Nonprofit organizations are important because they allow for the community to extend a smile to someone who has less than themselves. Carolyn Rose Award
Your Story Concierge Services, Inc would like to introduce the Carolyn Rose Award for nursing excellence. The Carolyn Rose Award was created to recognize nurses who provide excellent care, are committed to serving others, continually show compassion, demonstrate courage by advocating for their patients, initiate communication with families and patients, and are competent care providers. By nominating someone for this award, you agree that nursing is a better profession because of the nominee.
This year the Carolyn Rose award will be given to a nurse whose characteristics demonstrate excellence. The recipient will be a long-term care nurse from the CSRA and exemplifies a commitment to service. The recipient will demonstrate care, compassion, courage, communication, commitment, and competence in nursing as Carolyn Rose did. Nominations will be open starting March 15, 2021, and will close on April 19, 2021. The winner will receive their award on May 12, 2021, in person.
Carolyn Rose Sapp, born November 24, 1947, was a native of Millen, Georgia. She graduated from Augusta Technical College as a practical nurse. After migrating to Detroit, Michigan, she returned to school to obtain her Registered Nurse degree. Carolyn Rose worked in several hospitals in the Detroit area to include the Holy Cross, Henry Ford, and then Mercy Hospital. Her heart was compelled to help others and travel the world. After she visited Jerusalem, Carolyn Rose was ordained in the ministry and became an evangelist for God in 1998. Carolyn Rose was an advocate for serving others and giving a smile with a kind word to help someone who came across her path. With her infectious laugh, she could maneuver the most difficult situations to create the most remarkable outcome for everyone. The organization created this award to honor Carolyn Roses’ memory, her gift for serving others, and dedication to nursing. The recipient of this award will continue giving compassion, caring for others, serving others, and nursing dedication.

Jennifer was nominated by a co-worker who stated: "I am nominating Jenn for this award for her awesomeness. She comes into work with a smile on her face, and greets everyone by name. She brings positivity, to the residents no matter what area she is working that day. Jenn gives 110%, making sure that residents are taken care no matter what. We were short CNA’s one day and she did her job passing meds and then went right in to help feeding residents and make sure residents were taken care of and helping out the CNA’s. She is very compassionate and is the epitome of a nurse."
Jennifer stated " I love nursing and caring for others and I do it with a smile because it is what I love."
